SIICP becomes 1st hospital in India to use Mayo Clinic developed M-Compass for Anorectal Manometry test

SMILES International Institute of Coloproctology is the first hospital in India to use anorectal manometry technology developed by Mayo Clinic USA.

mayo-clinic-siicp

Mayo Clinic is a nonprofit organization committed to clinical practice, education and research, providing expert, whole-person care to everyone who needs healing. Mayo Clinic is ranked number 1 in the United States on the 2018–2019 U.S. News & World Report Best Hospitals Honor Roll, maintaining a position at or near the top for more than 27 years.

Each year, more than 1.3 million different patients from all 50 states and from more than 150 countries are seen at one of the Mayo Clinic facilities. Mayo Clinic offers highly specialized medical care. Mayo Clinic researchers contribute to the understanding of disease processes, best clinical practices, and translation of findings from the laboratory to the clinical practice. SIICP is proud to be associated with Mayo Clinic and use their technology for better and fast healthcare in India.

The Mayo Clinic designed and manufactured M-Compass is an FDA approved anal manometry device. Using an M-Compass, the Anorectal Manometry Test (ARM) is performed to evaluate patients with fecal incontinence/chronic constipation.

Today millions suffer from fecal incontinence and constipation while access to manometry equipment is limited. Easier access to M-Compass can put many suffering from fecal incontinence/chronic constipation on the road to recovery and drastically improve their lives.

M-Compass

The M-Compass device is portable, wireless, and weighs only 3 lbs. It is easy to use, affordable and can be used in a doctor’s office. Conventional ARMs are huge, aren’t portable, and can only be used in a dedicated room. The M-Compass is also simple to setup, consists of only 3 components, and has built in software guidance while conventional ARMs are large complicated systems.

Prior to performing an ARM using an M-Compass, no bowel preparation is required and there is no need to restrict one’s diet. Also patients who undergo an ARM using an M-Compass are not required to discontinue routine medication. An advantage of the Anorectal Manometry Test is, when it is used along with adjunctive tests, it can not only confirm a clinical diagnosis but also provide new information that cannot be clinically detected. It can also influence the outcome of patients suffering from incontinence/chronic constipation. Patients who have been tested so far using the M-Compass are highly satisfied.

Speaking about how people will benefit from the M-Compass Dr. Parameshwara C M said “This innovative technology will allow people across India to be tested more accurately than ever before. Using M-Compass an ARM can be completed in as little as 5 minutes. Of the millions across India who suffer from incontinence/chronic constipation, many will be greatly helped by M-Compass. We believe that bringing this technology to the masses so that millions can live more comfortable and happier lives is needed. We’re proud to be the first hospital in India to do so”.

SIICP is proud to add a milestone to our list and it is an honor to associate with Mayo Clinic, USA.

Factors responsible for increasing colorectal cancer in India

Dr. Parameshwara C M in conversation with www.thehealthsite.com about Colon cancer in India

Colorectal cancer in India

Over the past few decades, there has been an increase in the number of Indians suffering from colorectal cancer.

Main reasons for this are

  1. Hectic urban lifestyle Indians have adopted.
  2. As a part of this lifestyle more Indians today are eating out and often eating unhealthy food. Foods such as fast foods are high in fat and cause people to become overweight or obese. People who are overweight or obese are far more likely to suffer from colorectal cancer than others.
  3. often not getting exercise. Exercise is essential to remain fit and for the body to continue functioning optimally. Today much work is performed inside an office, so people in Indian cities don’t get enough exercise.
  4. Eating irregular meals. This means they eat at different times during the day, every day, putting the body’s rhythm out of synchronicity leading them to eat more at odd times and becoming overweight or obese.

Everything you want to know about Kshar Sutra

Dr. Parameshwara C M in conversation with www.thehealthsite.com

Kshar Sutra is an ayurvedic parasurgical treatment used to help manage anorectal disorders such as hemorrhoids, fistula-in-ano, anal fissure, and pilonidal sinus

Kshar Sutra is performed by first coating a surgical linen thread of gauge number 30 with Snuhi Latex 11 times, snuhi latex and Apamarga Kshara 7 times, and snuhi latex and turmeric powder 3 times. The patient is anaesthetized and a malleable probe with the surgical linen thread (called the Kshar Sutra) in its groove is passed through the external opening of the fistula to its inside and the probe is then carefully taken out with the Kshar Sutra intact. After this, both ends of the Kshar Sutra are tied together and replaced with a new one after a week. After this procedure, the tract is eventually cut and healed because of the removal of unhealthy tissue. This controls microbial infection and facilitates drainage of pus in the tract aiding in healing.

Our chief colo-rectal surgeon Dr Parameshwara C M

According to Dr Parameshwara C M, the therapy for Pilonidal Sinus is done in a similar manner while for haemorrhoids, a patient is first sedated after which the pile mass is brought out of the anal orifice using forceps and then the mass is given an incision at the mucocutaneous joint after which the pile mass is pulled and transfixed with Kshara Sutra at its base. Finally, the pile mass in the rectum is replaced with Yastimadhu Taila or ghrita.

What Ksharsutra helps treatment of colorectal disorders

Ayurvedic treatments such as the Kshar Sutra are extremely benign and don’t have any side effects. After the Kshar Sutra the results a patient experiences are permanent and benign. This makes Kshar Sutra an effective treatment for those suffering from haemorrhoids, fistula-in-ano, anal fissure, and pilonidal sinus. Those who undergo such treatment recover from the treatment in just 3-5 days after having stayed at the hospital for only 5-6 hours only 30-45 minutes of which are used to perform the surgery, ” Dr Parameshwar says.

An advantage of Kshar Sutra is that consultation for the surgery and the treatment is not expensive. The treatment is completely natural and there are few, if any, after effects from the surgery which makes many patients opt for this.

Kshar Sutra is beneficial to many patients. However, there are benefits to integrating this surgery with other techniques as well. This treatment should be supplement with home remedies such as eating food rich in fibre and consuming more water. Also while this surgery is an effective first line of treatment, if a patient’s condition worsens then advanced techniques such as DG-HAL RAR, MIPH, RFA, and IRC may be used.

Following are the excerpts from Dr. Parameshwara C M interview with Kannada popular newspaper Vijayavani. To read full interview click here.

Hemorrhoids are one of the most common problems that can cause swelling blood vessels in the bottom area i.e. anal canal. 50 percent of people above 50 years of age suffer from hemorrhoids. However, this problem can be lifted and controlled by lifestyle.

Dr Parameshwara C M

What's the reason?

As the age increases the risk of hemorrhoids is higher. Hemorrhoids appear in severe conditions such as severe constipation, acute diarrhea, continuous weight loss, difficulty in pregnancy or defecation.

What causes colorectal cancer? by Dr. Parameshwara C M

Dr. Parameshwara CM

what-causes-colon-cancer
As more than 90% of the cases of colorectal cancer are encountered in people over the age of 50 years.

Age and lifestyle are the main factors that cause colorectal cancer. Physical inactivity, increased alcohol consumption, smoking, and irregular and unhealthy diet are the major risk factors. Some of the recent studies have shown that a diet with a low fibre and high-fat content can lead to colorectal cancer, which is a common trait of western fast food. Apart from these, drinking water less than the recommended amount can also lead to the deterioration of natural bowel system and induce the development of polyps. Age also plays a crucial role, as more than 90% of the cases of colorectal cancer are encountered in people over the age of 50 years.

What are the symptoms?

At times, people may not see any symptoms during the early stage of colorectal cancer or they may experience some changes in their bowel habits. The symptoms of rectal cancer include diarrhea or constipation constantly occurring for nearly four weeks or more, rectal bleeding or blood in stool, abdominal pain, stomach cramps, or gastritis. Colorectal cancer can also co-exist with hemorrhoids or fissure. The affected person has the feeling that bowel doesn’t empty properly. These adversities further lead to weakness, fatigue and weight loss. The intensity of these symptoms also varies depending upon the stage of cancer and its location in the body.

If patients experience intermittent abdominal pain, irregular bowel movements, loss of weight and passage of blood during defecation, they should seek a doctor’s help.

What are the other types of digestive diseases?

Diseases of the digestive system can affect the whole body if not addressed in time with proper treatment or medical counselling. Some of the common digestive diseases include GERD, gall stones, Crohn’s disease, ulcerative colitis, irritable bowel syndrome, haemorrhoids, etc. Here are some of the most common problems:

  • Irritable bowel syndrome (IBS)Repetitive stomach pain or irritating bowel for a long time can be a symptom of irritable bowel syndrome. Generally identified with symptoms like chronic constipation, diarrhoea, or bloating, IBS is a very common disease among 10-15% of the world’s population.
  • Gallstones: These are hard deposits in the gallbladder, and are usually a result of excessive cholesterol or waste in bile. Medication or surgery is the only solution for this disease.
  • Gastroesophageal reflux disease (GERD): GERD is one of the most common digestive diseases that is often characterised as burning pain in the middle of the chest or upper abdomen. Commonly named as ‘heartburn’, it occurs when stomach acids back up into oesophagus creating an acid reflux. Doctors usually recommend antacids and analgesics.
  • Ulcerative colitis: A celiac disease that’s related to gluten sensitivity, ulcerative colitis causes bowel inflammation, mainly in the large intestine.
  • Haemorrhoids: These result in inflammation of the blood vessels in the digestive tract and blood in the stool.

Apart from these, the formation of polyps under uncommon health conditions may lead to dreaded diseases like stomach cancer, liver cancer, pancreatic cancer and colorectal cancer.

Busy lifestyle and irregular food habits have severely impacted human health. India, in particular, has encountered an increase in digestive diseases over years.

constipation-treatment-doctor
ODS is common among women aged between 45 and 60. Between 2% to 30% of people in the Western world are afflicted with chronic constipation, and 30% to 50% of them have ODS.

City doctors cured a Mysuru woman of chronic constipation, for which she had been consulting doctors for 26 years.

Hema (name changed) got the disorder treated recently at Smiles International Institute of Colo-Proctology (SIICP) in Mathikere.

Doctors at the institute diagnosed her with obstructive defecation syndrome (ODS), also known as ‘iceberg syndrome’, and performed a Stapled Transanal Rectal Resection procedure on her.

Hema is doing well after the surgery.

ODS is common among women aged between 45 and 60. Between 2% to 30% of people in the Western world are afflicted with chronic constipation, and 30% to 50% of them have ODS.

‘Complex disease’

Doctors describe ODS as a complex disease that is difficult to diagnose and needs to be treated in multiple ways.

Patients get a sensation of blocked defecation and will find it harder to empty the rectal contents. This will make them endure prolong and unnecessary strain.
